Description

The postholder will have a key role in strengthening relationships and governance across the system to help deliver the objectives of the ICB, including transformation of services, reducing health inequalities and delivering integration.

The postholder will be a key leader within place-based partnerships.

Areas of responsibility will be shaped by the priorities of the Place but may include working on areas such as clinical pathways, population health, collaborative-led programmes, education and talent management.


Specifics:

Collaborative working to deliver planning priorities including embedding new ways of working between primary, secondary and community care and to improve services, delivering better population health outcomes.

Building and developing key relationships and communication networks across primary and secondary care in the locality.

Work closely with colleagues from primary care networks, local care partnerships, LMC, GP federations, acute providers, community services and service users to co-design service improvements.

Taking into account operational planning guidance and the NHS Plan identify and align priorities for recovery in relation to system working and transformation Work with Clinical and Professional Directorate colleagues in collaborative ways to deliver system priorities with Place teams.

Contribute to locality Protected Learning Time agenda setting and delivery.
