The Discharge Lounge are recruiting for a Band 2 Healthcare Support Worker to join our team working alongside the Discharge Lounge Sister/Charge Nurse and Operational Coordinator to facilitate and coordinate patient flow through the acute setting to the Discharge lounge


You will provide and deliver a high standard of nursing care recognising and meeting the specific needs of the patients/clients.

This could include undertaking duties as per patient care plan (e.g. Hygiene duties, providing 1:1 support to a patient.


Should you be successful in the recruitment and selection process, you will commence employment on a permanent contract working 31.5 hours per week which could be a combination of 3 x 10.5 Hours shifts a week.


  • As requested, attend SAFER ward round and contribute, prioritise and identify tasks to support Patient flow.
  • Present findings to the patient flow Manager in real time, to include any escalations and report bottlenecks or delays.
  • Update and manage the maintenance of swift plus to ensure it reflects the current discharge plans and the ward bed availability.
  • Collect/deliver TTO (to take out) medications.
  • To support the effective and timely flow of patients on the ward, support plans for admission or discharge.
  • With support of MDT identify patients for Discharge Lounge.
  • Facilitate transfer of patients from wards/ED/Assessment area (PULL model).


The Discharge Lounge Health Care assistant will be joining the team at an exciting time, supporting the discharge lounge, as we continue to explore alterative locations and models of delivery at Torbay and South Devon NHS Foundation Trust.


The role is to support the effective and timely flow of patients from wards into the Discharge lounge providing high standard of nursing care recognising and meeting the specific needs of the patients.
