Working in partnership with NHS continuing healthcare teams and case management companies our enhanced level of specialist clinical support enables individual clients to remain at home even when the level of care required is highly complex.


We are dedicated in providing care for those patients who are too ill to look after themselves or be looked after by carers or family members and who would otherwise be admitted to, or have to remain in hospital, but who do not require 24-hour nursing care.


We are looking for an ITU or Registered nurse with extensive experience in:

  • Tracheostomy
  • Ventilator
  • Gastrostomy
  • Cough Assist
  • Suctioning
  • Epilepsy

Some of the duties required will be:

  • Communicate effectively with patients, carers and all members of the multi-disciplinary team.
  • Maintain accurate and legible patient records in line with professional and organisational requirements this includes contemporary paper records and electronic patient record systems.
  • Actively participate and contribute to team meetings, handover and MDTs.
  • Initiates health promotion activities, as appropriate, with individuals, patients and carers.
  • Responsible for a caseload for your shifts, with the support of a named Team/Clinical Leader.
  • Keep up to date with professional requirements, health care developments, clinical evidence, National Service Frameworks for practice, National and Local priorities.
  • Responsible for completing all mandatory training.
  • Responsible for the security, care and maintenance of equipment ensuring standards of infection control and safety are maintained including equipment loaned to patients.
  • Travel in the community so must be a car driver/owner
- expenses will be covered.


For this position we would also require:

  • Experience of community working
  • Registered General Nurse (Adult)
  • Valid NMC registration
