Description
City 1 (Norwich) Community Mental Health Nurse/Occupational Therapist/Social Worker - £3,000 Welcome Bonus

The team takes pride in being a highly supportive team that wants to get the best out of each other and make a difference to service users with moderate to severe mental illness.

We'd like you to be able to demonstrate a sound understanding of your role as a Lead Care Professional and have an understanding of how service users may enter our service and how we plan interventions to support recovery.

You'll need sound assessment skills; our secondary mental health service cares for individuals with severe and enduring mental health problems such as bipolar, trauma, personality disorder and schizophrenia.

When you join us, you can expect to enjoy plenty of opportunities for joint working, case formulation, training and supervision.

Team members are regularly involved in whole service delivery development, including Day service, Psychological Therapies and joint working with Primary Care.

You'll be working alongside a multi-disciplinary team and part of your role will be to undertake and support clinicians with assessments, undertake the role of the duty worker and manage medication administration.

You'll regularly be working with external agencies as part of a service users care, so being able to work well in a team and having excellent communication skills is vital.

You can expect a thorough induction and training / preceptorship period, as well as opportunities for ongoing development.

You can expect a manageable case load and a supportive management team to ensure you're able to work independently but support is never far away if needed.

The typical job roles and responsibilities will include:

  • Supporting and undertaking assessments depending on experience
  • Attending regular MDT meetings and liaising with colleagues from other services (both internally and externally such as Social Care, Crisis Resolution and Home Treatment Team and third sector organisations)
  • Supporting a caseload via face to face and telephone contacts
  • Involving service users, families, and carers in care planning and decision making
  • Participating in the duty role and the teams FACT model for providing short term stepped up care.

Here at NSFT we pride ourselves on being a welcoming, talented, friendly and supportive team who like nothing better than sharing experiences and learning from each other.

In addition to ongoing training and development opportunities, we are committed to providing an environment in which you can thrive.
