Job Title:

Team Leader - Supported Living and Adult Outreach


Contracted Hours: 30 Hours or could be a shared role of 2 x15 hour contracts

Rate of Pay:
£12.75 increasing to £13.86 following completion of probation period


Accountable to:
Registered Manager


Purpose of role:


Provide leadership and day to day supervision within your support worker team acting as a positive role model at all times and providing a needs led service to adults with a learning disability by adopting a person centred approach that fosters self-awareness, personal growth and gives each person the strongest voice with regards to decision making and lifestyle choices.


Main Duties and Responsibilities

Service Provision:


  • Ensure that the support team fully understand the service needs and support plans of each person they support, and that the service provided reflects the individuals needs and wishes.
  • Support individuals with regards to decision making and lifestyle choices enabling them to take control of their own lives, increasing independence.
  • Advocate for service user's and ensure all health and social needs are explored and met.
  • Develop positive relationships with families and carers.
  • Ensure thorough record keeping and evidence progress against desired outcomes.
  • Administer medication in line with Carlisle Mencap's medication policy.
  • Work professionally with outside agencies, including but not limited to commissioners, Adult Social Care and health professionals.
  • Contribute to/attend reviews and multidisciplinary meetings.
  • Take the lead on supporting individuals to reach their full potential and engage with activities.

Leadership:


  • Through positive role modelling, you will encourage and support the team to behave and communicate in a manner that aligns with Carlisle Mencap's Social Care Values and Behaviours.
  • Motivate, support and mentor the staff team.
  • Ensure there is positive, open and structured communication within the team.
  • Promote good team working and staff conflict resolutions.
  • Provide effective supervision to the support workers.
  • In conjunction with the Service Manager, organise rotas and ensure the shifts are covered safely.
  • Develop and implement personalised support plans ensuring they work alongside Positive Behaviour Support Plans.
  • Induct new staff into the services ensuring they have the knowledge required to support the service users with their needs and wishes.
  • When required, support the manager with the recruitment and interview process.

Safety and Administration:


  • Be willing to undertake training specific to the role.
  • Be proactive in identifying risks and acting accordingly.
  • Maintain clear and detailed records.
  • Undertake finance and medication audits as required.
  • Ensure all medication in services is stored and administered in accordance with the Medication Policy.
  • Work within and promote the Safeguarding Policies of Carlisle Mencap and Cumbria County Council Adult Social Care
  • Ensure the support team receive inhouse training specific to the individual's needs.
  • Support the staff team in the administration of medication through observations and ensure all competencies are completed.

Organisational Compliance:


  • Participate in relevant training to update and enhance knowledge base.
  • Be willing to undertake RQF level 5 in Health and Social Care.
  • Adhere to all company policies and procedures.
  • Participate in regular supervision with your line manager.
  • Undertake any other duties as may reasonably be required. This may include transfer to another location or service.
  • Participate in the out of hours On Call rota.
  • Uphold the good reputation of Carlisle Mencap.
  • Maintain confidentiality at all times (with the exception of where there are safeguarding concerns).

REQUIREMENTS:


Qualifications

Desirable
RQF Level 2 in Adult Care or other qualification related to the role e.g. Care Certificate


Experience

Desirable
Previous experience working with individuals with a learning disability in a residential or community setting


Skills

Essential
Good communication skills

Ability to keep accurate records

Ability to direct, develop and support everyday living and community skills such as cooking, cleaning, using public transport and shopping

Ability to assist individuals with monies and help manage finances


Desirable
Ability to use sign language e.g., Makaton

Full driving licence

Familiarity with local community
