Main responsibilities Utilise population health intelligence to proactively identify and work with a cohort of patients to deliver personalised care Help people manage their needs through answering queries, making and managing appointments, and ensuring people have good quality written or verbal information to help them make choices about their care, using tools to understand peoples level of knowledge, confidence in skills in managing their own health Support people to take up training and employment, and to access appropriate benefits where eligible for example through referral to the Social Prescribing Team Provide coordination and navigation for people and their carers across health and care services, working closely with our Social Prescribing Team and other primary care professionals Support the coordination and delivery of MDTs within the PCN Actively work towards developing and maintaining effective working relationships both within and outside the practice Liaises with other stakeholders as needed for the collective benefit of patients including their GP, Nurses, our Social Prescribers, Pharmacists, Pharmacy Techs and other healthcare professionals Develop excellent working relationships with the wider service networks including the voluntary sector, adult social care, hospitals and community pharmacists Communicate effectively with service users and their families/carers, other staff and members of the public Manage and prioritise workload on a daily basis Other responsibilities To act at all times in an anti-discriminatory manner To be able to plan and respond to workload according to operational priorities To undertake any training required for the role including mandatory training To take responsibility for self-development on a continuous basis, undertaking on-the-job training as required
