If so, West Berkshire Council has an exciting opportunity for you to join our Adult Social Care Service as a Social Care Practitioner for Continuing Health Care.

As a Social Care Practitioner, you will play a crucial role in assessing and reviewing the social care needs of individuals receiving Continuing Health Care.

You will work closely with service users, their families, and a range of healthcare professionals to ensure the delivery of high-quality care and support.

Your key responsibilities will include:

Conducting comprehensive assessments of social care needs
Developing and implementing care plans
Providing advice and guidance to service users and their families
Collaborating with external agencies and healthcare providers
Monitoring and reviewing the effectiveness of care arrangements

To excel in this role, you should have:

A relevant professional qualification in social work or a related field
Experience in working within a social care setting
Sound knowledge of relevant legislation and best practice
Strong communication and interpersonal skills
The ability to work autonomously and make informed decisions
