If so, West Berkshire Council has an exciting opportunity for you to join our Adult Social Care Service as a Social Care Practitioner for Continuing Health Care.


As a Social Care Practitioner, you will play a crucial role in assessing and reviewing the social care needs of individuals receiving Continuing Health Care.

You will work closely with service users, their families, and a range of healthcare professionals to ensure the delivery of high-quality care and support.


Your key responsibilities will include:

  • Conducting comprehensive assessments of social care needs
  • Developing and implementing care plans
  • Providing advice and guidance to service users and their families
  • Collaborating with external agencies and healthcare providers
  • Monitoring and reviewing the effectiveness of care arrangements

To excel in this role, you should have:

  • A relevant professional qualification in social work or a related field
  • Experience in working within a social care setting
  • Sound knowledge of relevant legislation and best practice
  • Strong communication and interpersonal skills
  • The ability to work autonomously and make informed decisions
