Role Summary Perform preliminary arterial assessments of patients triaged by Vascular Scientist or Vascular Nurses. Responsible for the booking and administration of these patients. This may entail making minor modifications to the treatment plans within prescribed parameters and reporting back on progress. To be responsible for the booking of all patients to the claudication clinic and informing the patients.

Ensuring effective use of clinic capacity, improving patient pathway and maintaining/improving efficient resource management. To undertake preliminary audit work for the Sussex Vascular Networks National Vascular Registry entries.

To Maintain the Trusts Abdominal Aortic Aneurysm (AAA) surveillance database (used to track patient pathways ensuring timely scans and reviews in line with national guidance prior to reaching surgical threshold).

Responsible for the collection, recording, reporting and use of data as determined by the Directorate Management Team, ensuring high quality data and timely reporting.

Ensure patients are booked according to local protocols, ensuring GPs and Vascular Consultants are informed of results.

Key Working Relationships The post holders key relationships will be with:

Other NHS Trusts, NAAASP, National Vascular Registry coordinators, other network administrators, GPs, community care teams and CCGs Main Duties and Responsibilities Communication Arrange and book all appointments, (face to face and telemed) for the claudication clinics at the RSCH and PRH.

Ensure all patients receive appropriate appointment letters.

Cancel and re-book appointments to suit patient needs. Ensure that appointment slots are fully utilised. To deal with large volumes of day-to-day enquiries from patients and trust staff in a polite and professional manner. Telephone non-attenders to ensure patients are not lost to follow-up.


Implement clinical and technical services to patients and provide information in relation to the care of those individuals and groups.

Request specific data from the vascular consultants for the VNS to enter into the NVR for amputee patients requiring interpretation, assessment, tact and persuasive techniques to enhance the patients ability in accessing the rehabilitation programme, in a sensitive situation, throughout the hospital and outpatient setting.

Provide concise, legible and accurate data/records maintaining confidentiality at all times. Ensure all records are uploaded onto the appropriate electronic notes.

Maintain accurate records for research and audit purposes.

Use own judgement and initiative to help resolve capacity issues with the Vascular Nurse Specialists Service Delivery and Improvement Maintain documentation on all patients seen in, ensuring all are on the correct clinical information systems.

Demonstrate the activity of the clinic and teach the performance ABPI (Ankle Brachial Pressure Index) measurement to colleagues, including trained nurses, junior doctors and medical students this will be on a regular basis.

Compile and update patients personal details on CareFlow and amend their files accordingly.

Create files for patients seen in the claudication clinic, ensure diagnostic test results are available before the appointment. Arrange and book all scan appointments, ensure all patients receive appropriate appointment letters. Cancel and re-book appointments to suit patient needs. Ensure that appointment slots are fully utilised.

Compile and update patients personal details on CRIS. Generate consultant review request according to the SOP. To arrange and book nurse assessment clinics for all AAA patients in local surveillance to align with NAAASP. Ensure all clinics are recorded on CareFlow.

Ensure that all patients receive appropriate appointment letters, re-book appointments as required. Be responsible for and demonstrate competence in the use of medical devices appropriate to the care setting. Perform equipment checks and prepare equipment ready for patient use. Participate in service reviews and actively put forward ideas for service improvement.

Adapt own practice in line with changes and developments of the service. Maintenance of the most effective and economical use of all supplies and equipment, demonstrating an awareness of budgetary control. Maintain stock control. Patient Care Delivery Communicate any changes in the patients condition to the appropriate clinician.

Ensure the safety and privacy and dignity of the patient is maintained.

Ensure that all patients having major arterial surgery are added to the National Vascular Registry, adding basic demographics, requesting medical notes to allow VNS to submit clinical data.

Undertake autonomous vascular assessments using ultrasound techniques to generate the data used to plan patient care. Ensuing all results are reported back to the initial referrer.
