Job Description - Support Worker (temporary Contract to cover Maternity leave)

Centre Manager
Hours: 20 hours per week, 10.00am to 14.00pm, Monday to Friday
Salary:
' £11.00 per hour. A Work Place Pension Scheme is in place to which the employer contributes 5%

Purpose of the Job
- To care for the physical, emotional and social needs of the clients using a personcentred approach
- To observe and promote the client's choice, independence, dignity, privacy, fulfilment and other rights
- To create and maintain good professional relationships with clients, their family and friends and other stakeholders
- To adhere to all regulatory and statutory obligations and Acredale House policies, procedures and guidelines
Job Responsibilities/Duties
- To provide personal care and support to clients with a wide range of needs
- To contribute to the care planning process for clients in consultation with other staff
- To ensure that care practice is nondiscriminatory and that clients personal beliefs and preferences are appropriately acknowledged
- To be aware of and responsible to the needs of older people with mental health problems e.g. dementia
- To encourage the independence and motivation of client's through identified activities
- To assist clients with their medication at the agreed level of support as detailed in their support plan
- To assist clients with personal care
- To maintain good communication and develop effective working relationships with clients
- To assist with the transport of clients to and from their homes to the centre in the minibus
Recording and Reporting
- To maintain detailed accurate records in respect of care and medication and activities undertaken
- To regularly review client support plans, acknowledging changes
- To protect the confidentiality of all information relating to clients and not divulge information to anyone who is not authorised to receive it.
- To promptly report any issues concerning the care, support, wellbeing or behaviour of the client to the manager and update records accordingly.
- To continue to monitor where concerns have been reported or recorded
- To recognise the signs of abuse and immediately report abuse or suspected abuse to the manager
General
- To dress appropriately and wear personal protective equipment when required
- To attend and participate in regular care work team meetings and any other relevant meetings
- To attend in house and external training as required
- To observe all health and safety regulations and raise any concern to the manager.
- Any other duties requested by management which are withing the scope of the post
Job Types:
Part-time, Temporary contract
