Description
To support the Pharmacy Team (currently Pharmacist, Pharmacy Technician and Prescription Administration) in delivering their services, with particular focus on hospital discharges, triage of queries and tasks, phone consultations with patients, carers and pharmacies and the co-ordination of the prescriptions process.
It is expected that the Pharmacy Medication Reconciliation Clerk will also have input with the prescription administration team in on-going reviews for improvement of prescriptions processes and policies (governance), ongoing monitoring and implementations of suggestion and audit.
