This is an exciting opportunity to work with our newly appointed SRO Primary Care to support development and delivery of the North West London (NWL) ICS Primary Care and wider Out of Hospital Strategy.

The post holder will play a key role in working with the North West London primary care community to develop and deliver new models of Access to primary care and establish Integrated Neighbourhood Teams, working closely with Borough Based Partnerships.

The post holder will provide a short term boost to capacity in the current NWL team whilst the new ICSs operating model is refined and embeds.

Priorities for the post holder include:

Support to PCNs to provide a foundation for population health management and the development of Integrated Neighbourhood Teams
Development of a new model of Access to primary care that protects capacity for prevention and pro active care and provides an integrated reactive care pathway at neighbourhood level
Development of the GP OOH service ensuring that it is fully integrated with 111, UTC and extended access hubs as part of the Reactive care pathway
Support the development of a Primary Care Performance Framework to demonstrate operational performance, delivery of ICS objectives and impact on the wider system
Support development of the NWL primary care team and joint working with Borough Based Partnerships.
Day to day support to the SRO Primary Care
