Description

Following development of our Frailty strategy and recent expansion of the department to 22 consultant geriatricians, we are looking to appoint an Advance Clinical Practitioner to help lead and develop post-acute care and build a culture of nurse-led community interface services.

This is a very exciting time to work within a progressive, dynamic and rapidly expanding department. We have a large operational footprint within the trust, running over 200 of the c1000 inpatient beds.

Our multidisciplinary team participates in acute admissions, rapid access clinic, inpatient geriatric medicine, ortho-geriatrics, major trauma, perioperative medicine, movement disorders, neuro-palliative care, community & care home medicine.

Most recently, we have begun to develop a virtual frailty ward and early supportive discharge models of care for frail older people.

The post-holder will work closely with the wider multidisciplinary team, and will be supported professionally.

Specifically, there will be a need to learn to independently manage acute and chronic illness occurring in these older patients.

Furthermore, an important part of the role will include complex communication and the development of independent practice in Advance Care Planning.
