An excellent opportunity has arisen for a dynamic pharmacist to join the Medicines Optimisation Team in Southwark.

In the changing NHS Landscape, the Medicines Optimisation Team is continuously developing collaborative engagement with partners across the borough and the wider South East London environment.

Through commissioning, the team promotes the development of safe and effective provider-based medicines optimisation functions, to enable clinical and cost-effective use of medicines for our population.


We are an established and inspired team and have a long history of successful working relationships with our local stakeholders.

As part of the Medicines Optimisation Team, you will be working closely with GPs and other clinicians to shape healthcare and NHS commissioning locally.

With excellent interpersonal and communication skills, you will work as part of Southwark Medicines Optimisation Team to improve medicines use for the people of Southwark, who have a rich cultural, social, and economic diversity.

You will contribute to promoting and implementing medicines optimisation initiatives within primary care, focusing on cost-effective and quality prescribing.

You will also support the delivery of a portfolio of national (QIPP) by monitoring locally agreed prescribing targets and assist with the development of local dashboards.

The team is committed to training and development, and the post holder will therefore be involved in the direct supervision of trainee pharmacists at our central London office.

The post holder will work as part of Southwark Borough's Medicines
Optimisation Team to develop, manage and deliver medicines optimisation
in general practice within Southwark ensuring best value, reduced harm and improved outcomes from the significant investment in medicines.
The post holder will advise on medicines aspects of service redesign in line with ICB priorities and objectives.
The post holder will lead, develop and deliver specific medicines optimisation projects and services.


Southwark Borough is part of South East London ICB and is made up of all GP surgeries in the area.

Our job is to make the most effective use of our allocated budgets, to best meet the health needs of people in Southwark.

Our main providers are Kings College Hospital NHS Foundation Trust, Guys and St Thomas' NHS Foundation Trust and South London and the Maudsley NHS Foundation Trust.

Responsibilities:

Medicines Optimisation

  • Provide pharmaceutical expertise to the Southwark member practices,
stakeholders and patients by appraisal of current clinical evidence and best practice.

  • To support and mentor band 7 pharmacists as appropriate, maximising their medicines optimisation skills e.g. at practice level, guideline development etc.
  • Contribute to medicines optimisation decision making at relevant meetings and support the Senior Prescribing Advisers on specific areas of work e.g. PGDs, nonmedical prescribing, service redesign/care pathway development work and interface projects.
  • To develop, manage and deliver support to GP practices, to optimise cost
effectiveness, quality and safety of prescribing in primary care, in line with
local and national guidelines.

  • Provide a clinical medicines update (annual review) to practices, following up and documenting specific actions as required
  • Provide feedback to GP practices on progress with medicines initiatives,
prescribing targets and clinical audit.

  • Support the work streams in primary care e.g. Prescribing Improvement
Scheme and other local schemes to improve medicines optimisation

  • Support all prescribers within the primary health care team, and contribute to practice reports, newsletters and the local website.
  • Provide expertise and leadership on medicines in one or more therapeutic area as agreed by the Associate Director Medicines Optimisation (e.g.
cardiovascular drugs, respiratory, antimicrobials).

  • Support the clinical governance and patient safety agenda in all areas of
medicines management.

  • Support implementation of national safety alerts.
  • Assist in the development and implementation of guidance, including
national guidance, Integrated Medicines Optimisation Committee (IMOC)
guidelines, shared care guidance, NICE guidance in order to achieve national and local prescribing standards.

  • Promote interprofessional working in the delivery of medicines optimisation across the interfaces of care.
