Description
1. Gateway to Care provides advice and information to people regarding Social Care and the intermediate tier of health. This ensures the well-being of individuals and their families by taking proactive approach to preventative services and staying well. 2.


Through using enhanced interpersonal and communication skills provide a professional and knowledgeable response to people and a range of professionals, resolving in a courteous and timely way the range of queries received by Gateway to Care 3.

Screen and assess the needs of people who contact Gateway to Care analysing information gathered and make decisions based on that information.

Make onward referrals where appropriate or provide directly access to some services. 4.


Make decisions that are in accordance with relevant policy guidance about eligibility for services, deploying the correct use of legislation aimed at maintaining the independence of vulnerable adults.

Identify and report issues of Safeguarding, Mental Capacity or significant concerns taking appropriate action in line with legislation and West Yorkshire multi-agency safeguarding policy.

Proficient user of IT to maintain up to date records, including the computerised Client Information System and other computer-based systems.

7. To provide high quality written and verbal documentation which will be clear, concise, and logical. Ensure that documented information is accurate, up to date, relevant and reflect the departments involvement with the person.


  • Provide a customer focused service, always maintaining a professional manner, including when difficult situations arise. 9. Form links with voluntary agencies and community groups to ensure better outcomes for people, encouraging people remain independent and connected to their own Communities.
