We have a fantastic opportunity for a Health Visitor to work within the Ilkeston team in the Erewash Locality, for 30 hours per week, delivering services to children and families as part of a Section 75 contract.


The post holder will provide health visiting services to families in the community team caseload by delivering the Healthy Child Programme at a community, universal, targeted and specialist level.


The post holder will also be able to undertake face to face contacts on a daily basis to respond to client needs.

The post holder will

  • Deliver the public health role as a SCPHN Health Visitor working with communities and partner agencies and delivering the core contacts within the Healthy Child Programme.
  • Provide health visiting services to families in the community team caseload by delivering the Healthy Child Programme.
  • Manage a defined active caseload and take responsibility for Safeguarding Children issues within that caseload.
  • Have continuing responsibility for the assessment of individuals needs and the development, implementation and evaluation of programmes of care.
  • Prescribe according to assessed individual need from the Nurse Prescribing Formulary.
  • Manage a team of health visiting support staff.
