Description

The post holder will work with key members of primary and secondary care to provide education, advice and support to patients with heart failure and will support the Cardiac Rehab Service in enabling patients with heart failure and other cardiac conditions to improve their cardiovascular health.

The post holder will be responsible for education and service development within the heart failure team and act as a resource for advice for healthcare professionals, patients and carers.
