Responsibilities and Duties
Completing care assessments for clients in their homes.

This includes meeting with prospective clients to discuss their needs, creating person-centred care plans that include in-depth risk assessments and tasks lists for carers.


Promote the highest standards of care and service delivery - ensuring quality standards and governance are maintained within all aspects of the business.


This will be done by:

  • 1) Liaising with next of kin and other key contacts, including health professionals, throughout this process.
2) Completing wellness visits and spot checks for existing clients to ensure quality of care provided meets best practice

3) Completing regular care plan reviews for existing clients.

Take responsibility (alongside On-call Supervisor) for ensuring the emergency on-call provisions are covered outside normal office hours - participating in the on-call rota (1 in every 4 weekends)

Develop and manage relationships with clients, their families and other professionals involved in our client's care

To work alongside the Registered Manager ensuring that all quality standards, complaints and comments are dealt with promptly.

Work with Directors and marketing team to seek opportunities to strengthen professional relationships. Promote, grow and drive new care packages and identify new business development opportunities and networks.


We expect the community care manager to be spending at least 2-3 days a week in the community, with the remaining 2-3 days based in the main office.


About you:


Having a full UK driving license and your own vehicle is essential. A fully enhanced DBS certificate is also a requirement for the role.
You are a natural team-leader who has an excellent eye for detail. You will have proven experience in leading, training and managing a team and excellent knowledge of the care industry. You will be commercially forward-thinking and able to operate in a fast-pacedwork environment.


Essential:


  • Have strong experience working within a busy domiciliary care environment
  • Be a strong team player with the ability provide leadership and management and in a supportive, positive and rational manner
  • Highly organised approach to work with strong attention to detail
  • Have strong interpersonal skills and be able to communicate confidently with all stakeholders written and verbal
  • Strong IT skills proficient at MS Office i.e. Excel, Word, Outlook; and care systems (Birdie experience is desirable but not essential)
  • Ability to prioritise with a flexible approach to work
  • Sound and up to date understanding of CQC assessment criteria
  • Have proven experience managing a service in a similar role in a busy home care agency environment (ideally with experience of managing a livein service)
  • Ideally hold (or working towards) a Level 5 Diploma in Leadership for Health and Social Care (or former equivalent qualifications)
  • Please note: a willingness to complete the Level 5 qualification if not already qualified is a must and full support willbe given
