Do you hold a professional clinical qualification such as registered nurse, allied health professional etc with experience of working in the field of children, young people and families?


We are looking for an experienced and highly motivated Specialist Practitioner for our Children and Young People Complex Care services across Hampshire and the Isle of Wight (HIOW) to join our growing team.


This is an exciting time to join the service with opportunities to work with a wide range of stakeholder's and in partnership with other agencies.

We are an expanding team working Monday to Friday, office hours and no weekends or shifts.

Children and Young People (CYP) referred to the CYP Complex Care service will receive a high-quality service ensuring that the child, family, and carers are at the heart of a personalisation approach.

We work closely with adult Continuing Healthcare to deliver the NHS All -age Continuing Care vision.

Ensuring patients receive the care they need, in the right place and at the right time is the top priority for the Hampshire and Isle of Wight Integrated Care Board

Support the delivery of a high quality, operationally safe, clinically led service through delivery of robust systems to implement policy and guidance relating to the NHS Continuing Care processes.


To utilise the national assessment tools to ensure preparation of good quality needs portrayals and assessments based on the National Framework Decision Support Tool, attend panels, support the appeals process and present cases to the Multi Agency Resource and Health Funding Panels.


Manage packages of care through effective case management ensuring high quality care provision, working collaboratively with families, NHS, Local Authority provider multi-disciplinary teams, brokerage team and the care provider market.


Contribute to monitoring and reporting key performance indicators to ensure Hampshire and Isle of Wight Partnerships of Integrated Care Board (HIOW) is kept updated as part of performance assurance of NHS Continuing Care.


Support the Clinical Lead to manage the clinical team administrator and contribute to the training and development of all staff regarding policy and guidance around the NHS Continuing Care policy and process.

This will encompass explaining the process to patients, families, health, social care colleagues and care providers, ensuring that the process is transparent and clearly understood by all parties.
