Working hours Monday-Friday 9am-5pm.

  • Schedule and coordinate resident appointments and followup visits
  • Assist in the coordination of care plans for patients, including referrals to specialists and other healthcare providers
  • Communicate with patients, families, and healthcare professionals to ensure continuity of care
  • Maintain accurate and uptodate patient records
  • Provide support to patients and their families by answering questions and addressing concerns
  • Collaborate with healthcare team members to ensure efficient and effective delivery of care
  • Assist in the coordination of home health services, medical equipment, and supplies as needed
  • Process referrals to medical professionals
  • Create and review person centred care plans
  • Undertake on call duties.
