Locum Band 6 Community Physiotherapist - Central London

We have a fantastic opportunity for a locum Physiotherapist to join a leading teaching hospital, based in Central London. They are seeking a qualified Physio with previous Community experience.

  • 4 month (minimum) locum assignment to start as soon as possible
  • Full time hours (Monday

Position Requirements

Job Description:
To improve the transition of patients from acute services back to the community.

The post holder will work in partnership with colleagues in the acute sector, social services, intermediate and primary care services to ensure that appropriatepatients from the acute setting are assessed and transferred in a safe, timely and seamless manner into the community.

The post holder will ensure effective discharge practice takes place for patients with discharge planning needs by facilitating multi-agencyand multidisciplinary team working and being responsible for coordinating the CIS Home 1st workload.

To identify/case find and pull potential CIS patients in partnership with acute colleagues ensuring that as soon as patients are medically stable they aretransferred to the appropriate community service.

To independently undertake assessments of health and social care needs of patients in order to plan community services required.

To support and enable the process of safe discharge from secondary to CommunityServices and assist in bridging the gap from hospital to home.

To proactively promote a seamless and multidisciplinary approach to health and social care provision.
