Derby City Council is looking to recruit a Community Care Worker on a fulltime permanent basis. You will deliver the very best service and outcomes to adults within the Learning Disability Assessment Team.

As a team we support individuals with learning disabilities who may currently be living in a residential/nursing care home, supported living, independent hospital or with their family.

You will be assessing and reviewing customer support packages with a view to promoting choice and control using person centered approaches.

You will also be supporting some individuals to move from residential care to a life in the community providing advice and guidance.
