Temporary Grade 6 Service Co-ordinator opportunity has arisen in the Community Reablement Service for 12 months.37hours Monday to Friday 9am - 5pm based at Macclesfield Town Hall including days working in Westfields Sandbach or Leighton Hospital.
Reablement is a short-term service supporting the Home First Agenda of hospital discharges and hospital avoidance and includes referrals from the Community Social Work Teams.

The role of the Service Co-ordinator will include viewing and actioning referrals working on our Social Care IT system, loading the customer data onto our local computerised systems, completing all the allocation of scheduled visits for each customer and templating.
Working closely and liaising with the Hospital and Brokerage Team when ongoing needs are required to source outside packages of care.
The service also provides Crisis Response and hospital avoidance with A+E/District Nurses. The Service Co-ordinator also attends daily Integrated Discharge Team meetings to discuss all care packages and capacity.
The completion of the weekly staff rotas and checking daily to ensure all customers have their Reablement calls in place, ensuring continuality which is paramount to each individual customer's journey.
This is fast paced busy post and the job holder must be enthusiastic, reliable and have excellent communication skills and able to work on their own initiative or as part of a team and have good reading and writing skills and an eye for detail.
