Our Cardiothoracic Surgery department within our Essex Cardiothoracic Centre have a fantastic opportunity for a patient pathway coordinators to join its warm, friendly and supportive team.
This is a 12 month fixed term with potential to become permanent working within a specific clinical area to facilitate the smooth running of the patient journey by ensuring that co-ordinated and streamlined administrative processes revolve around the patient and their individual needs.
- Work within a specific clinical area to facilitate the smooth running of the patient journey by ensuring that coordinated and streamlined administrative processes revolve around the patient and their individual needs.
- Have overall management for a number of Patient Pathway teams including; attendance management, appraisals and general monitoring of performance and compliance with Trust, Divisional and Speciality standards.
- Support the clinical team, Operational Manager and other Divisional managers in ensuring the speciality are able to deliver safe and effective services whilst also ensuring compliance with contractual access targets e.g. the 18 weeks Referral To Treatment target.
- Provide Crosscover for other a Patient Pathway coordinator to ensure a consistent and effective administration services is maintained.
Your role will be to co-ordinate, monitor and initiate appropriate changes within administrative processes along the patient pathway to ensure the patient's journey is managed efficiently, smoothly and in accordance with targets and agreed timescales.
Yo will utilise computerised tracking systems to locate case notes and investigation results, liaise with teams outside of the speciality e.g.
GPs where appropriate to ensure a seamless patient journey, ensure patient information is maintained and updated on the appropriate patient information system and monitor work lists Consultant workflow and escalate to Operational Managers if necessary.
You will be required to monitor patient attendance to appointments and re-arrange/book further appointments as required, have knowledge of TCI's and pre-assessments and achievement of agreed KPI's.
