Do you want to join an exciting service focused on directly improving patients care in Swindon? The Navigation Hub has been established to do just that.


We are looking for someone who is passionate on supporting the development of this service alongside the team of Clinical Assessors and can bring their enthusiasm and skills with to support our patient's pathways.

The team is part of a wider project to co-ordinate care in Swindon within the Swindon ICA Coordination Centre.


The Navigation Hub is service within Great Western Hospital which aims to support patients attending the right place of care for their medical needs in a timely fashion.

It aims to reduce unnecessary hospital attendances and reduce the time patients are waiting for and in ambulances.

It further attempts to reduce multiple assessment and moves ensuring the patient receives the right care at the right time in the right place delivered by the right team.

The successful applicant will work closely with the Clinical Lead, Navigation Flow Nurse and Clinical Assessors of the Navigation Hub and across our organisation to reduce the number of attendances to hospital and to streamline the patients to the appropriate team.


This post is key to supporting our patients and team by ensuring the correct records are completed in a timely fashion, supporting the development of this new service alongside our clinicians and developing your own skills.

The successful applicant will be responsible for compiling a variety of reports and working alongside the Navigation Hub Call Handler,

Responsibility for Patients

  • Provides nonclinical advice, information or guidance to patients, their relatives or carer
  • Locates, obtains and tracks medical records for both elective and emergency admissions
