Job purpose/summary:

The Care Co-ordinator within the PCN will be expected to assist the PCN teams to deliver the Network Contract DES, support delivery of quality and safe services within the PCN hours and services.

To support the running and monitoring of clinics including diary, annual leave and sickness monitoring. To add in clinics according to agreed PCN need. To submit monthly financial claims in a timely manner.

To work closely with all members of the ARRS team.

There is the potential to be the shared decision making sponsor for the PCN and Care Navigation Champion The Care Co-ordinator will act as a focal point of communication between patients, and other clinical and non-clinical members of the practices with regards to the activity of the PCN and its required specifications.

Main Duties and Responsibilities To facilitate the PCN Access Plan including Rota Cover Involvement in complaints management process directly related to PCN activity Population Health priorities to ensure needs are being met for SEC through peer information sharing, To deal with PCN queries from staff and patients, To support and deliver PCN promotion/ website updates, with close liaison with SPLW, H&WC, MHT To work closely with all ARRS team members, to have sight of rotas, hours, HR monitoring and clinics To be responsible for the timely return of all network information, reporting and statistics as required locally or nationally with support from the Digital Lead Collection and analysis of practice activity information relevant to the network and the reporting of this to the PCN Governing Body and external bodies as appropriate.

Liaising with practices to ensure that, as necessary, all managers and clinicians are aware of administrative and clinical requirements of the PCN and your role within it Support the PCN Manager in meeting timescales/deadlines for audits and written returns to ensure that the PCN meets quality standards and receives the designated funding.

Scheduling, administration, attendance and minuting of PCN meetings.

Support the PCN Manager and Senior Finance Business Partner with PCN related administration, claim submission and budget preparation, forecasting and monitoring of all PCN related income and expenditure.

Operational and project management of all identified or directed PCN level projects or projects that benefit all PCN practices.

Respond to queries from patients requiring general information relating to the PCN:

procedures, registration, care preferences, queries on services and navigation to services Monitoring the usage of appointments ensuring that clinics are well utilised and appropriately used.

Monitoring effectiveness of the PCN systems and reporting any problems or variations to the designated Manger.

Supporting design and implementation of system changes and efficiencies through promotion and training To support admin duties within the hubs utilising PCN teams to ensure effective and smooth running of services, supporting training and development of current PCN staff To support the role out of the Reception Competency Framework and promote active care navigation Develop and maintain a Personal Learning Plan and maintain current knowledge, including mandatory training and offers of PCN roles development Involving in Project implementation discussions Implementing the agreed discussions by Creating Staff Rota, filling rotas, Creating appointment books in EMIS Maintaining Spreadsheets related to Projects/Appointments/Payments/Reports Agreeing on Report Format to submit to ICB or other stake holders Point of contact on any submissions to ICB that has to be done in Timely manner Approving Invoices for the Staff who has worked for PCN or any other Projects that were taken by PCN Monitoring Clinical Tasks in EMIS Updating Ops teams or operational manager about availability of appointments every week Providing training to users on how to use PCN EMIS Creating users in PCN EMIS and updating Smartcards Monitoring Extended Access/hrs appointments utilisation General Duties At all times to behave in a professional way that encourages quality care, the development of a team spirit and fosters continual improvement across the PCN.
