Responsibilities:

  • Provide direct care and support to individuals in a community setting
  • Assist with daily living activities, including personal hygiene, meal preparation, and medication management
  • Develop and implement individualized care plans based on client needs and goals
  • Monitor and report changes in client's physical and emotional wellbeing
  • Collaborate with a multidisciplinary team to ensure coordinated care
  • Advocate for clients' rights and promote their independence and inclusion in the community
  • Maintain accurate and uptodate documentation of client interactions and progress

Job Types:
Full-time, Part-time, Permanent


Salary:
£10.75-£11.00 per hour


Expected hours:
No less than 16 per week
