Therapeutic Practitioner based in
Flintshire with our Young People's Drug & Alcohol Team (Flintshire Sorted)


Hours:3 days 0.6 WTE


Salary: 26,395.88 per annum pro rata (£15,837.53)


Holidays: 25 days per year plus 8 public holidays (pro rata)


Based at:
Flintshire Sorted Office, County Hall, Mold


Responsible to:
Flintshire SORTED team manager


Job Purpose:

Flintshire Sorted provide a comprehensive service to young people across Flintshire including universal, targeted and specialist services.

This post will provide targeted and specialist services to support young people with complex needs who are at increased risk of using substances and for those identified with a substance misuse problem.

The post holder will provide a range of therapeutic interventions to meet the needs of the client group.


Responsibilities:


  • To have commitment to and demonstrate an undertaking of equality of opportunity and its relevance to different sectors within the community.
  • Must demonstrate empathy towards Adferiad Recovery's beneficiaries.
  • To carry out all other tasks commensurate with the role.

Client Work

  • To undertake specific young people's assessments re substance use and related issues in respect of referred clients under the age of 2
  • To provide a range of therapeutic interventions to meet identified needs of the client group.
  • To establish, maintain and then disengage from relationships with the client group.
  • To work with young persons in a time and task specific manner on a variety of issues likely to impact on their wellbeing, including their housing, current employment, training and education status.
  • To provide a range of substance use interventions including education, harm reduction, motivational interviewing, relapse prevention and exit strategies as appropriate.
  • To ensure best practice with regards to assessment and interventions for young people with drug and alcohol issues as well as emotional wellbeing issues.
  • To provide a specialist support and consultancy service to other professionals in respect of individual clients they may be working with on substance misuse use and emotional healthrelated matters.
  • To provide a range of drug and alcohol treatment and support services to vulnerable young people involved with the Youth Justice Service including cases referred by the Prevent & Deter Panel / Bureau.
  • To work in partnership with Flintshire Sorted colleagues to maintain a seamless, continuous and effective service.
  • To provide specialist support and training for other professionals.
  • To provide statistical information as required.
  • To support a smooth transitions to adult services as appropriate.
  • To work in collaboration with other adult services such as SMS and CMHT.
  • To assist in the ongoing promotion and development of Flintshire Sorted in the local area.
  • To ensure the health and safety of yourself and others who may be affected by the work undertaken.
  • To provide a service that accounts for the assessment of risk.
  • To participate in the planning and delivery of group work with targeted groups of young people in all settings.
  • To work as part of a team with other Flintshire Sorted workers and relevant agencies in developing and delivering Drugs Awareness / Prevention programmes for young people where appropriate.
  • To raise awareness within the target group of the risks of becoming involved in substance misuse.
  • To positively work with and support young people at risk of drug misuse.
  • To work as part of a team in contacting hard to reach young people in a variety of settings.

Administration

  • To work within the Flintshire County Council / Flintshire Sorted/Adferiad policy framework especially in relation to the Child Protection procedures, Curriculum Development, Equal Opportunities, Financial Regulations, Health & Safety and Staff Development Policy.
  • To ensure that the Flintshire County Council Drug and Alcohol Policy and detached/outreach/mobile guidelines documents are adhered to.
  • To maintain regular contact with Adferiad, in particular the Service Manager
  • North Wales.
  • To maintain up to date records of current client work, case notes including electronically on CareWorks (Client Information System)
  • To follow appropriate procedures when making referrals to other services and agencies.
  • Prepare/contribute to the preparation of individual client and service reports.
