Description

This role is based within Platfform's Supported Housing scheme within the Vale of Glamorgan.

The scheme is provides tenancy related support to persons with complex and/or enduring mental health needs as evident either through their support history, because they are under the care of secondary mental health services, or both.

The role holder will be expected to provide support and mentoring to Support Workers in the development of individual support plans (ISPs) and risk assessments (RAs), as well as take lead during crisis support.

Compliance with and maintenance of all necessary administrative and reporting systems will be required.

We work with people experiencing challenges with their mental health, and with communities who want to create a greater sense of wellbeing in the places that they live.
