Description

This is an exciting opportunity to help towards NHSE Community Liver Health Check programme the post holder will work within a hospital and community setting along with challenging environments such as prisons and homeless hostels assisting with the increase of referrals, screening and ongoing follow-up where needed.

They will work as part of the team as well as independently.

The post holder will be responsible for implementing nursing care programmes e.g. Fibroscanning in line with Trust procedures as requested by the medical/nursing staff.

They will also have a key role in supporting those diagnosed with liver disease to subsequently engage with ongoing liver health care follow up.


The post holder will perform tests such as Fibroscan and will participate in multi-disciplinary team meetings to support the team to make decisions on test results.

They will be expected to maintain patients' privacy, dignity, and confidentiality at all times, and in all situations. They are to be an effective member of the Hepatitis service and contribute to the objectives.


Working under the guidance of the Lead Specialist nurse and the Hepatology team the post holder will implement care packages which will ensure a smooth pathway for patients with suspected liver disease from diagnosis through to treatment and follow up.


The postholder is required to communicate with members of the public to provide and receive complex, sensitive information such as explaining to patients what tests are going to be made and the reasons for doing this.

The postholder will communicate regularly with members of the multi-disciplinary team and other health care providers to inform of issues relating to the patient's wellbeing


The postholder will work collaboratively with others such as outreach workers and nurses working within the hospital setting and outreach clinics to solve problems using persuasive, motivational and re-assurance skills as required.
