Location:

West Northumberland Primary Care Network (PCN) consists of 10 GP practices providing services to approximately 66,000 patients

Practices include:

Bellingham Practice, Branch End Surgery (Stocksfield), Burn Brae Medical Group (Hexham), Corbridge Medical Group, Haltwhistle Medical Group, Humshaugh and Wark Medical Group, Prudhoe Medical Group, Scots Gap Medical Group, Sele Medical Practice (Hexham) and White Medical Group (Ponteland).

We also provide services to 2 other practices within West Northumberland, although not part of the PCN - Riversdale Surgery and Adderlane Surgery, both in Prudhoe.

We are looking to recruit three Care Home Care Coordinators to work within our network alongside our multidisciplinary healthcare teams, playing a key role to support our Enhanced Health in Care Homes (EHCH) service.


Job Purpose:


You will be the key link for the people whose care you are supporting, ensuring continuity of care by acting as a point of contact for care home staff and professionals as well as family members and residents.

The MDT is a key part of the role, and ensuring the identification of people who need additional support to manage their care, as well as supporting the development of personalised care and support plans for those patients.


Working closely with GPs and practice teams, as well as care home matrons and care home staff and other professionals.

Making sure appropriate support is made available to people, and enabling people to access services.

Providing coordination, secretarial and administration support for the EHCH service.


Key Responsibilities

  • To work closely with GPs and other primary care professionals to manage a caseload of patients, acting as a central point of contact to ensure appropriate support is made available to them and their carers;
  • Overall responsibility for arranging Multi-Disciplinary Team ("MDT") meetings, including the weekly Care Homes MDT meetings (virtual or face to face), managing the meeting agenda items; ensuring that all new referrals are identified, and information circulated to team members in advance of the meeting;
  • Support with identification of patients in need of review and collation of information of patients needing full MDT review.
  • Take minutes of MDT meetings and disseminate; chase progress against actions identified in these meetings and ensure follow up where necessary;
  • Liaise with all clinical and nonclinical members in the MDT to ensure effective MDT function;
  • Manage reporting required and associated within the Direct Enhanced Service specifications for required services; and
  • Bring together all the information about a person's identified care and support needs and exploring options to meet these within a single personalised care and support plan.
  • Helping residents and families focus on what matters to them, ensuring they feel care is personalised and that their voice is heard at professional meetings.
  • Provide coordination and navigation for people and their carers across health and care services, alongside working closely with social prescribing link workers, ARRS staff and other primary care roles.
  • Liaise with care home staff and patients and carers within the home when needed.
  • Undertake accredited training as set out by The Personalised Care Institute
