Utopia Care are looking to recruit an experienced Care Coordinator. If you have experience in this position and are looking for a new challenge, get in touch Due to the requirements of the role, you must be able to drive and have access to your own vehicle

RESPONSIBILITIES:
Answering telephone enquiries

Support the Registered Manager in the recruitment of carers.

Processing new client referrals

Preparing appropriate documentation for assessments and completing them with clients

Allocation of carers to clients and ensuring all visits always covered

Administration of all Individual person centred care plans

Maintaining client and office files and ensuring that they are always up to date

Ensuring all documentation and records are kept up to date at all times

To be flexible and able and willing to complete care visits that have not been covered

To ensure compliance with all office systems and procedures

Participation with the on call phone on a rota basis

Production of reports and management information as required

To complete audits as required

Part time contract available 16-20 hours per week

Up to £13 per hour
