Description

This role will support the coordination of care for cancer patients through their cancer pathways from pre-diagnosis to end of treatment.

This role will be focussed around the telephone support of the cohort of patients who are referred to Frimley Health from local GPs, internal services and other trusts.


This role will be guided and supervised by the Macmillan Information and Support Officer in collaboration with the Head of Nursing Cancer and Macmillan Information & Support Manager.


The post holder will coordinate the care and support given to cancer patients, their families and health care professionals who contact the Macmillan Cancer Navigator service at Frimley Health.

The remote cancer navigation service will act as a telephone based single point of access for people affected by cancer from pre-diagnosis to end of treatment and support.

The post holder will also support recording of national audit data for cancer patients in collaboration with clinical teams to support various elements of the cancer pathway.


Supporting Cancer Clinical nurse specialists by taking on administrative roles related to supporting Holistic Needs Assessment, documenting, tracking and recording, all patient and service users interactions.

The post holder will also provide information to patients and service users regarding support available during their cancer pathway.

Providing holistic support and information and signposting to existing support in the hospital and community.

Working alongside registered practitioners to improve care for people with cancer, working closely with Clinical Nurse Specilialists and Allied Healthcare Professionals.

KEY RESULT AREAS:

To make an effective contribution to reaching the Trust's vision, strategic objectives and key work programmes for cancer patient

To coordinate care for patients from referral throughout their pre-diagnosis and treatment pathway

To provide a 'touch point' for patients and GPs with the hospital to ensure two way communication during treatment

To provide guidance, contact and support for patients as they journey through our hospitals

To ensure all appointments are notified in a timely manner providing an excellent patient experience providing required information

Signposting to other support services

Support people affected by cancer to self-manage as part of SSMP

KEY TASKS & RESPONSIBILITIES

Development of navigation pathway for patients pre-diagnosis, through treatment and end of treatment

Provide a single point of contact for patients throughout their treatment

Responsible for contacting and organising appointments and information and results based on decisions made by a registered practitioner,

Coach patients and carers in the safe use of any equipment to be used at home in line with guidance provided by a registered practitioner

Coordinate face to face appointments as per MDT instructions

Comment on proposed changes to policies and procedures

Support and contribute to audit processes, governance, research, clinical research trials and service development

Report any protocol variance to MDT and CNS

Responsible for validation and updating of database(s) required for national and local reporting

Support the planning/inviting/organising of informative events for patients


The post holder will have a good knowledge and understanding of patient pathways within cancer, including the diagnostic staging investigations and treatment options available.
