An exciting and rewarding opportunity has arisen for a suitably qualified, enthusiastic and passionate Health Care Support Workers (HCSW) to join the Bridgend East Primary Care cluster.


The East Cluster includes five GP surgeries, covering a population of approximately 77,729 patients and works collaboratively alongside other Primary Care Contractors such as community pharmacies, Dental and Optometry colleagues, District Nurses, the Third Sector, other Allied health care Professionals and the Local Authority to support the health of our population.

The cluster is fully committed to meeting the health needs of the population it serves and by working collaboratively with a 'multi-disciplinary team' approach allows us to achieve our goals.

We believe this can bring huge benefits to our population with improved access to services closer to home and new and improved services and models of care for the population.

The HCSW will work within the cluster multi-disciplinary team and with the cluster primary care nurse.

The HCSW will also carry out Home Visits on behalf of the registered GP, to include nursing and residential care on behalf of the practices.
