Frailty Virtual Ward Service Hours 7 days a week, 8am - 8pm

An exciting opportunity has arisen for enthusiastic and motivated HCA to join the South East Essex Frailty Virtual Ward service.
These important posts will work alongside a team of Consultant Geriatricians, Community Nurses, Therapists, and Health Care Assistants in the provision of the Frailty Virtual Ward working in the community, in the patient's own home.
You will be part of a high profile service to help alleviate current pressures in the NHS.The service will reduce demand for inpatient beds and on Emergency Departments and optimise patient experience by enabling them to receive safe clinical specialist care in the comfort of their own home.

What is Staff Bank?
Our EPUT NHS staff bank is an entity managed by the trust that hires clinical and non-clinical healthcare professionals to take on shifts at our trust hospitals and community settings.
Here at EPUT we maintain our own bank of specialist staff to ensure that we are able offer safe and effective care at all times.
All our permanent staff are automatically enrolled onto the staff bank however this does not mean you have to work any additional shifts, but the option is there for you if you wish.
If you are joining our Trust in a fixed term role, please indicate on your New Starter Paperwork that you wish to join our staff bank.
FVW is an admission avoidance and early supported discharge team from hospital available 7 days a week that will help patients stay at home when they are feeling unwell rather than be admitted to hospital.
The Band 3 Health Care Assistant will work as part of the FVW Team to deliver acute care in patients' homes, including the monitoring of observations, venepuncture and supporting patients with personal care and rehabilitation during an episode of acute ill health.
KEY RESPONSIBILITIES CLINICAL
It is accepted that at times, the HCA works without direct supervision and the Sister acknowledges accountability of all delegated care.
To manage daily workload efficiently and effectively, taking into account the needs and preferences of the patient.
To maintain practice to the standards set by the Care Quality Commission.
Work towards promoting admission avoidance from primary care to the acute Trust.
Provide care to a client group with acute and complex health care needs to ensure the highest standard of care is given which complies with current clinical guidelines and legislation.
To be familiar with the steps to be taken in medical emergencies and act accordingly.
Work with the FVW Therapists to support the delivery of holistic care and admission avoidance interventions.
Provide advice and support to home carers in the community on the FVW caseload
KEY RESPONSIBILITIES PATIENT / CLIENT CARE
To undertake through each patient's individual care plan, those duties delegated by the FVW Sister and therapists, which the Health Care Assistant has been trained and is competent and confident to perform
These tasks include:
- Personal care
Monitoring of baseline physical observations
Blood glucose monitoring
Urinalysis
Phlebotomy
Cannulation
Male / Female catheterisation
Wound care management
Pressure area care
Administration of nebulisers and medication
Collection of specimens
Administration of insulin (on completion of competencies)
Full training and competencies will be provided to ensure that the post holder has the appropriate skills to undertake these duties.
