Description

Under the supervision of the practice mentor/teacher, participates in the assessment of health needs and formulation of care plans for children, young people, their families and communities, this includes the implementation of programmes of care, offering support and advice to parents and families in the home, clinic, school or community setting e.g.

parenting skills, public health, group work and child health clinics.

In collaboration with the public health nursing team, identifies individuals and groups within the community where collaborative working and advice would forward the interests of public health and promote healthy living, in particular seeking to identify disadvantaged groups and address health inequity.

Offers advice and support to children, young people and adults in accordance with evidence based practice.

Safely delivers immunisations in line with policy and patient group directions.

Under the supervision of the Practice Mentor/Teacher completes the Common Assessment Framework, working with the multi-agency team in supporting children in need and those at risk of significant harm, including observing in case conferences and other meetings and contributing to reports where appropriate.

This includes learning about the role of lead professional or key worker for children and their families with the most complex needs.
