We are looking for an experienced, motivated and well organised individual with highly developed skills to join our specialist team of Community Matrons supporting the often complex needs of care home residents within Heywood Middleton and Rochdale.
This is an evolving service and we are looking for someone who is keen to be involved in service development as well as innovative patient care.
You must be able to communicate effectively at all levels, work autonomously and be able to demonstrate effective leadership.
You will provide high quality care in an appropriate and timely manner, which is responsive to patient need and proactive in the planning optimisation of long term condition management.
You will be supported through regular team meetings, 1-1 sessions, on-going education with access to a professional team of peers, ACPs and AHPs to provide mentorship and support.
- Registered Nurse
- A desire to learn and develop clinical skills and knowledge
- Excellent standards of care
- Ability to communicate and work collaboratively with patient's carers and other members of the multidisciplinary team.
