Description
An exciting opportunity has arisen to join the central RTT validation team as a pathway coordinator.

In this role you will work across a range of multi-disciplinary teams in the management of patients' treatment pathways.

This includes guaranteeing the quality of data across a number of databases within the Trusts' Patient Administration System (PAS) to ensure that patients are in the correct place with respect to their treatment pathway, and that relevant outcomes are correctly recorded and communicated.

Good pathway management is fundamental to the patient experience and services rely heavily on this role to ensure that any trends or issues in patients' treatment pathways are highlighted.

Therefore, the post holder will be expected to assess, analyse and interpret data, and show excellent communication skills to maintain regular contact with the team and present any key findings.

The Patient Pathway Coordinator (PPC) will work as part of the Elective Care Operating Framework team, a Trust wide framework initiated to look at the way elective care is managed across the Trust, and will be responsible for ensuring that elective care patient pathways are delivered in a timely manner, and that patient pathway records are accurate, up to date, and reflect national and local policies.

They will provide support to their allocated services in looking into appointment outcomes and care plans and ensuring the next steps are well planned for and communicated to patients as well as providing administrative support to the wider multidisciplinary team, including Consultants.

The PPC will work with the Patient Pathway Supervisor and service Business Manager to ensure that patient pathways are recorded accurately to enable a high quality, effective patient focussed service.

The post holder will ensure that Standard Operating Procedures are followed and support the Patient Pathway Supervisors and service Business Managers in ensuring that all performance targets relating to elective care are achieved.

The PPC will be responsible for providing regular updates on performance and the escalation to the Patient Pathway Supervisor and service Business Manager of any issues which hinder the smooth delivery of the patient pathway.
