Overview:


Following an expansion of the team, The Base are excited to advertise the role of Young Person's Resilience Worker - Transitional Lead Young Adults.


The Base Kirklees are a confidential drug and alcohol service for young people aged up to the age of 21.

We offer advice and support to young people in who are experiencing problems with drugs and alcohol. This support offered includes specialist care planned interventions, brief interventions, targeted group work, outreach, and training.

The service works in a holistic way and works closely with local partners in ensuring a multi-agency approach to supporting children and young people.


Responsibilities:


We are looking for a Young Person's Resilience worker who will hold lead responsibilities in relation to supporting the transition of young people into adulthood including throughout their journey from children's services into adult services.


The post-holder's focus will be to work proactively with young people to help address their substance misuse needs and alongside this, they will help forge strong working relationships and pathways with local adult support services including social care, drug and alcohol treatment, mental health and criminal justice, to help coordinate a joined-up approach to substance use and other identified needs.


The successful applicant with therefore possess excellent engagement skills and possess the ability to work in partnership with other services and share their knowledge through the delivery of training around substance misuse to other professionals.

The ability to work to a high degree of accuracy and meet deadlines amongst competing priorities is vital in this position.
