Description

We are looking for an enthusiastic person with excellent team working skills, who is keen to develop their knowledge and skills in this area.

You should be a competent and motivated practitioner with appropriate demonstrable experience. You will work as part of a multi-disciplinary team providing assessment and therapeutic intervention in the community

Promoting independence and reducing risks for adults living in Doncaster in line with the Care Act (2014)

Undertaking a person centred care act assessment to identify need taking the appropriate action whilst respecting individual values, cultures and diversity

Identifying needs and creating support plans to meet identified needs as well as determining eligibility for services in accordance with Care Act Guidance

Management of a caseload acknowledging that some individuals may require assistance from other professionals

Identification of risks or incidents of possible abuse including reporting this to the appropriate staff member in line with the Councils Safeguarding adults policy and procedure

Supporting and working with Qualified Social Workers and other professionals in cases which have become complex

Assisting in the development and implementation of the locality based community led support model using creative ways to support service users and carers

To provide an initial assessment and interviewing service for older people, vulnerable adults and carers.

The Community Care Officer will undertake where appropriate a Care Act Assessment as well as providing a care management service for individuals with non-complex needs in accordance with Council policies.

This role will report directly to the Advanced Practioner under the supervision of the Locality Team Leader.
