An exciting and new opportunity has arisen for an experienced individual to join the Royal London Hospital Transfer of Care Hub Team as a Band 5 Discharge Navigator.

Your experience may be as a physiotherapist, occupational therapist, nurse or an experienced discharge co-ordinator.


The Transfer of Care Hub's job is to prevent avoidable delays in patient's discharge by discussing, coordinating, and planning in accordance with current government discharge guidelines and acting as a problem-solving resource to the ward multidisciplinary teams, senior management, and external agencies as well as advocating on behalf of your patients.


We are looking for an individual who has the confidence to challenge decisions at all levels, experience of working in a fast paced and high-pressure environment, and the ability to recognise the need to escalate complex discharges to senior management to prevent discharge complexities.


To lead the discharge assessment and planning of patients on the wards, from advanced planning prior to discharge to the point of onward transfer, with a pro-active case management approach drawing on post holders nursing, occupational therapy, physiotherapy or discharge skills dependent on professional background.


To work collaboratively, as part of the Transfer of Care Hub team and with ward based multi-disciplinary teams and in-borough and out-of-borough community health and social care partners.

To agree complex discharge plans that reflect the ongoing care needs of patients, and respect their wishes.


To lead on co-ordinating early discharge so patients leave hospital within 24 hrs being declared Medically optimised and ready for discharge in order to minimise discharge delays, improve inpatient flow and reduce the possibility of clinical deconditioning.

To pro-activley identify and escalate any potential delays in discharge and lead on problem solving/case review meetings as required.


To promote safe and effective discharge practice throughout the Trust and act as a resource for expert advice and guidance as required.

The service operates 7 days a week from 08:00 to 20:00 including bank holidays.

If successful, please note you will be expected to work on rota'd weekends, bank holidays and late shifts as required by the service.

We are a 7 day service operating from 08:00 to 20:00 including bank holidays. If successful, please note you will be expected to work rostered weekends, bank holidays and late shifts.

You will possess an ability to prioritise both clinical and operational workload to support the delivery of acute services at the Royal London Hospital.

Your experience will support a pro-active approach to a dynamic and continuously changing environment as well as containing the necessary skillset to support our staff in a variety of settings.

This role is key in the delivery of operational performance and supporting our clinical colleagues and management teams to perform to a very high standard.


We require an effective independent worker who equally works well within a team environment, with excellent communication skills and who visibly enjoys the challenges that this role will provide; a good understanding of IT is preferred.

These individual qualities will provide the foundation for you to join one of the finest teams in London's newest Hospital and of course Europe's leading trauma centre.
