The Education, Health and Care Team performs Rotherham Borough Council's statutory duties in relation to special educational needs and disability (SEND) assessments and placements. We are seeking to appoint a full time Education Health and Care Plan Assessment Co-ordinator to work within the team (Band H).
The Service:
Coordinates statutory assessment and maintains Education, Health and Care (EHC) plans for children and young people aged 0 - 25
Provides planning and decision making with regard to educational placements and SEN provision including personal budgets cited in EHC Plans
Monitors and reviews education placements and provision for Children and Young People with EHC plans in Rotherham
Ensures statutory annual reviews take place and that education settings have relevant advice and guidance to be able to complete their statutory duties in this respect.
As a member of this team the successful applicant will play a key role in ensuring that all processes are carried out in accordance with the Children and Families Act 2014 and SEND Code of Practice 2015.
- Develop strong working relationships with services, agencies and providers to contribute to effective multi-agency working.
Work sensitively and collaboratively with parents/carers, children and young people by acting as the single point of contact and ensuring their views and wishes are considered throughout the process.
Maintain a sound knowledge and awareness of professional business support practice, systems and technologies and to utilise these in continuously improving the quality, productivity, responsiveness and effectiveness of the SEND service.
