Job Summary:

We are currently seeking a skilled and compassionate Adult Social Worker to join our team in Birmingham.

As an Adult Social Worker in a hospital setting, you will play a vital role in supporting patients and their families during their healthcare journey.

This full-time locum position offers flexibility in pay rate based on qualifications and experience.

Responsibilities:

Conduct comprehensive assessments of patients social care needs, considering their physical, emotional, and social well-being.

Collaborate with the multidisciplinary healthcare team to develop and implement appropriate discharge plans, ensuring a safe and smooth transition from hospital to community-based care.

Liaise with patients, families, and external agencies to facilitate access to community support services, including home care, rehabilitation programs, and social support networks.

Provide emotional support and guidance to patients and their families, addressing any concerns or anxieties they may have regarding their care and discharge process.

Coordinate and arrange appropriate packages of care and support, ensuring they align with patients needs and preferences.
Conduct regular reviews and assessments of patients progress and adjust care plans accordingly, promoting their independence and well-being.
Collaborate with community-based social care providers and agencies to facilitate seamless integration of care and support services for patients.
Document all assessments, interventions, and care plans accurately and in a timely manner, adhering to professional and organizational standards.
Ensure compliance with relevant legislation, policies, and regulations governing adult social care practice.

Qualifications and Requirements:

Bachelors or Masters degree in Social Work or a related field.
Valid Social Work England registration or eligibility to register as a Social Worker.
Proven experience in adult social work, preferably in a hospital or healthcare setting.
Sound knowledge of relevant legislation and policies governing adult social care, including the Care Act 2014.
Excellent assessment and communication skills, with the ability to engage effectively with patients, families, and healthcare professionals.
Strong problem-solving and decision-making abilities, with the capacity to work autonomously within a fast-paced hospital environment.
Ability to work collaboratively as part of a multidisciplinary team, fostering effective communication and information sharing.
Proficient in documenting assessments, care plans, and progress notes in accordance with professional and organizational standards.
Flexibility and adaptability to meet the evolving needs of patients and the hospital environment.
