Are you an existing or an aspiring Advanced Clinical Practitioner? Are you passionate about improving services for frail and vulnerable older people? We are seeking highly motivated experienced nurses or allied health professionals to join our existing team of ACPs covering pathway 2A and 2B rehabilitation beds in Ashbourne, Belper, Heanor, Ilkeston and Ripley.
If you have a passion for life-long learning, enthusiasm for supporting the development needs of others, excellent communication skills and a commitment to continuous quality improvement we would love to hear from you.
You will lead and monitor improvements to quality of care through clinical audit, evidence-based practice, clinical supervision, teaching and supporting professional colleagues and the provision of skilled professional leadership.
You will lead the implementation of relevant clinical guidelines into practice and contribute to the further development of the evidence base through research and audit.
If offered a trainee position you will commit to undertaking an apprenticeship programme comprising a 3-year MSc in Advanced Practice and the development of an ePortfolio to demonstrate your capability across the four pillars.
You will be uplifted to Band 8a on successful completion of the MSc, successful submission of their portfolio to a cross-organisational review panel.
Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.
We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people's own homes and via virtual consultations.
During the pandemic response we have led the implementation of public vaccination centres in collaboration with partners across health and social care.
