Description
The Dementia Support Advisor's primary role is o provide Information and Support to Unpaid Carers supporting People with Dementia. Supporting the Dementia Team leader setting up and running Dementia Cafes across the Borough. Assisting Team Leader in providing Training Programme for Carers in Dementia Awareness

  • Primary_
  • Assist People with Dementia and their carers in identifying their needs and providing information and guidance to help maintain their independence, improving wellbeing and putting carers in more control of their lives. _You will be the first point of contact in supporting Carers who are supporting someone with Dementia.
  • Build relationships with a range of local contacts, networking with health and care professionals and providing dementia support.
  • Liaising with statutory and community sector, publicising Harrow Carers and the work being carried out to support carers. In particular, efforts will be made to target black and ethnic minority communities
  • To engage other agencies in work to find unidentified carers.
  • To assist the Dementia Team leader in managing the dementia cafes across the Borough.
  • Encouraging carers to participate in and contribute to group activities and maximise their use of existing services and resources in the area.
  • To record clients' information in line with Harrow Carers' confidentiality policy and within the organisation's database
  • To prepare reports, letters etc. to support the work of the Project
  • To be able to work on their own initiative and report to the Dementia team leader where necessary.

  • To undertake course work relevant to duties and responsibilities as outlined above
  • To review work performance with the Dementia Team Leader and complete staff appraisal at least once a year or as required
  • To take part in individual supervision

General_

  • Any other duties required by the Dementia Team Leader to facilitate the efficient running of Harrow Carers
  • The post holder must at all times carry out his/her duties and responsibilities with due regard to Harrow Carers' Policies and Procedures including the Equal Opportunities Policy
  • Car Driver is a requirement of this role.
