Continuing Healthcare Specialist Practitioner Band 6

Are you a Registered Health Professional who is committed to providing personalised care to patients? Do you want to be able to use your skillset within a different, unique setting?


Previous experience with CHC framework would be desirable but is not essential as we can provide a tailored induction and training programme for you.

You will be fully supported through 1:1 supervision, a robust induction plan, and a friendly supportive team. This is a community based post and post holders will be expected to be able to travel across their designated locality area

The post holder will be responsible for co-ordination of assessments of patients to identify eligibility for NHS Funded Health Care including entitlement for funding in respect of NHS Continuing Health Care and Funded Nursing care.


The post holder will require good verbal and written communication skills to communicate with patients, their families and other professionals.

The production of accurate, detailed and comprehensive reports is essential.


The post holder will work closely with hospitals, community services and care providers to develop systems and processes to facilitate a smooth assessment and referral process.


The post holder will review the clinical needs of the patient with regard to appropriateness of the placement, level of care being provided, safeguarding and entitlement to NHS Health Care.

Ensure accurate and open communication and co-ordination with a range of organisations and individuals.


Be a key member of the team as well as supporting effective communication and stakeholder management, both internally and externally.
