Change Grow Live are a charity dedicated to the belief that we can make a difference to our Service Users lives, offering support and respect in a safe environment, treating each user as an individual and working with them to find the right treatment and care options.


A fantastic, new opportunity has arisen to become a
Rough Sleepers and Homeless Recovery Co-ordinator**, supporting rough sleeping individuals throughout their treatment journey and successfully supporting their move on into suitable housing arrangements.

We are hoping this role will be for you.

Stevenage


Hours:37.5hrs per week


Salary:
£27, £31,529.41


Allowances:
Outer London Fringe Weighting of £683.99

Responsibilities:


About the role:


  • Establishing effective communication and referral pathways those individuals who are homeless and in contact with other health and care agencies
  • Work closely with the local authority, Health & Social care sector, partners and any provision linked to Homelessness and improve joint working, attending multidisciplinary meetings when as and when required
  • Supporting service users from point of entry into the service and through their treatment/recovery journey
  • Providing screening, assessment, and recovery planning and onward referral
  • Coordinate the transfer of any accommodated out of area individual. This may include referral to and liaison with their new drug and alcohol service and pharmacy and provision of bridging prescription until an appointment and prescription is provided. Promoting carer, service user and community involvement
  • Providing advocacy for access to partnership services
  • Working with service users to support social (re)integration, enabling them to lead meaningful and purposeful lives: promoting recovery, resilience, peer support and selfdetermination

About you:


  • Prior knowledge and experience of working with people who are homeless, sleeping rough or sofa surfing
  • Have a strong knowledge and understanding of substance misuse issues
  • Experience of engaging with a wide variety of individuals with a variety of backgrounds and needs
  • Be an excellent communicator, both verbal and written with good IT skills
  • Proactive and able to work on own initiative with excellent time management and prioritising skills
  • Have a good working knowledge of mental health interventions, services, and good practice
  • Be a supportive team player with strong interpersonal skills with the ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholders
  • Ability to manage change successfully in a way that prioritises the needs of service users
