The Positive Lifestyle Team in Central locality are seeking a skilled registered mental health nurse or social worker to join us in the role of Homeless Link Worker under the Changing Futures Initiative.
This is a new post so will provide an excellent opportunity for a practitioner who wishes to support service development, has a positive outlook and always puts the service user first.
Link working and developing relationships with other services (eg Foxton Centre, housing, social care and probation services) is essential and it is expected that the practitioner will be autonomous and proactive with a special interest in the unique needs of the homeless population in the Central Locality (Preston Chorley South Ribble and West Lancashire).
