We are looking for a motivated individual to join our community engagement and resettlement service in Hereford to provide housing support to individuals with complex mental health support needs as they step down from supported accommodation and rehabilitation settings into independent accommodation within the community.

About the role

  • Provide a comprehensive package of support to assist individuals to manage the transition from Lifeways SIL schemes, hospital and/or other supported accommodation into an independent tenancy in the Community.
  • Support the people we support to comply with the terms of their tenancy. Provide appropriate support where they do not.
  • To assist the people we support to Liaise with the landlord over any repairs/replacements needed.
  • Provide support to develop life skills and skills for independent living, for example, managing budgets, supporting a healthy diet.
  • Proactive awareness of the circumstances and needs of the people we support, with a willingness to provide support as needed; accompany them to interviews/meetings when required, record relevant information in personal files and liaise with staff and management regarding individual progress.
  • Hours are based on a rota working 24 hours per week that will include evenings and weekends including participating in the oncall rota on a week on/ week off basis.
  • The role will be home based, working in the community and in peoples own homes

About you

  • You will have an understanding of the needs of people with Mental Health needs and its effects
  • Able to monitor clients' physical/mental health and liaising with specialist services as appropriate.
  • Able to work effectively under guidance and on your own initiative
