Contract: 4 months Fixed Term - Full time
An opportunity has arisen for a Cancer Pathway Tracker to work within the Cancer Performance Team to undertake the tracking of patients through their cancer pathway.
We are responsible for submissions to Cancer Waiting times, The Cancer Registry, Peer Review and National cancer audits and Datasets.
Confidence in operating IT systems is essentialWorking hour requirements for this will need flexibility but normal working hours are 09.00 to 17.00 Mon-Friday.

MDT Co-ordination/TrackingTo organise and attend the multidisciplinary team (MDT) meetings.
To prepare appropriate lists of patients to be discussed at each MDT (and distribute throughout the team) from information received from a variety of sources including clinicians, medical notes, electronic patient records and previous MDT lists.
To prepare the MDT decision sheets prior to the MDT meeting with patient demographics, results of investigations and other relevant clinical information.
To ensure that the MDT are fully aware of the progress of each patient in relation to the 31 and 62 day target dates for treatment both via MDT Meetings and individually via Operational Management and Clinical Nurse Specialists to ensure proactive patient journey management.
other MDT co-ordinators, imaging, histopathology, chemotherapy, palliative care, other Trusts, etc.) for the preparation of the weekly MDT meetings; to ensure that all required information and clinical data (including notes, x-rays, test results etc) are available for discussion in a timely manner.
To ensure that any decisions taken during the MDT meetings on clinical action requiring administrative organisation, such as scans, appointments and surgery dates are booked and confirmed in a timely manner; to follow-up with appropriate departments and clinical nurse specialists to ensure such appointments are made appropriately.
To ensure that the decisions taken at the meeting are fully documented in the patients notes as a means of tracking and monitoring.
To file the patient's MDT decision summary and other documents into case notes promptly; to keep a copy of the decision summary for future use and for audit purposes.
To ensure all information relating to the MDT decision is recorded in the Somerset Cancer Register system (either during the MDT meeting or retrospectively); to liaise with clinicians to ensure such information is accurate and timely.
To ensure all appropriate audio/visual equipment is available in MDT or SMDT meetings for recording of the MDT decision on the Somerset Cancer Register system.
To organise the Specialist Multi-Disciplinary Team (SMDT) meetings in the same manner as described above for MDT meetings; to inform clinicians at other Trusts across London Cancer of any changes to the meeting time or venue; to inform Whittington clinicians and other Trusts of any changes to the SMDT format as appropriate.
