Referral Facilitator - Lincoln, United Kingdom - NHS Lincolnshire Integrated Care Board

The post holder will:
- Work as a member of the NHS Lincolnshire Integrated Care Board (ICB) Referral Facilitation Service (RFS) based within the Elective Activity Coordination Hub (EACH)
- Work as a member of the CCG Elective Activity Coordination Hub Team
Your main duty is to receive electronic referral for patients, undertake an admin check to ensure all relevant information is present, schedule the patient for clinical triage and then liaise with the patient by telephone to book them the most appropriate appointment.
The service works to strict deadlines and requires you to handle confidential, complex and sensitive patient's clinical information on a daily basis.

The Referral Facilitation Service forms part of the Elective Care Hub which is responsible for navigating patients and providing choice in their journey between presenting to their GP and them requiring specialist consultation and/or treatment.
